Eligible Dependents

Coverage under Group Accident Expense is available for a spouse, insured children and dependent grandchildren as defined in the policy/certificate (may vary by state):

     A spouse is the person to whom the primary insured person is lawfully married to and, if also an insured person under the certificate, was named on the enrollment or other form of application for the certificate as the primary insured person’s spouse, or who was added to the certificate at a later date. No more than one spouse may be insured at any given time.

     Insured children means the primary insured person’s or their spouse’s natural children, stepchildren, foster Children, adopted children or children placed as foster children or for adoption, who are younger than age 26.

     Dependent grandchildren means the primary insured person’s grandchild under 26 years of age, who is legally dependent on the primary insured person.

The application must include all family members to be covered. If the employee is applying for coverage for themself only, family members should not be listed on the application.

 

Dependents Who Become Eligible After the Issue Date

A spouse may be added to coverage after Assurity approves the application and required premiums are paid.

An insured child or dependent grandchild born to the primary insured person is covered from the moment of birth. Insured children or dependent grandchildren under age 26 adopted by the primary insured person or placed in adoption or as a foster child with the primary insured person are covered when adopted or placed. Assurity must receive written notice (including the child’s name, gender and date of birth) and any premium due for such child within 31 days to continue coverage beyond those 31 days.
