Childhood Critical Illness Rider (Form No. R G1718C; not available with Prime plan) (HSA compatible)

The Childhood Critical Illness Rider provides a lump sum benefit for each insured child and dependent grandchild upon diagnosis of certain additional specified illnesses, conditions and procedures. The amount payable is the percentage for each specific critical illness specified in the chart below multiplied by the selected benefit amount.

Policy provisions relative to the Additional Diagnosis Benefit apply to this rider.

Eligible Persons:  Insured children and dependent grandchildren

Benefit Amount:  Same as selected for the policy for insured children or dependent grandchildren; each critical illness is payable once per lifetime for each insured person
