Eligible Employees and Dependents

To be eligible for coverage, employees must be:

    actively working at least 20 hours a week (with Assurity approval, fewer hours may be accepted depending on employer’s benefit eligibility guidelines, however we will not go below 15 hours per week),

    performing the duties of their primary occupation currently and for the last 90 days (whether with current employer or past employers), and

    employed at their current employer for at least 30 days (some industries may require a longer period at Assurity’s discretion).

Those not eligible for coverage include:

employees with incomes substantially based on commissions,

1099 employees, and

retirees.

The following other individuals are eligible for coverage under a Group Health product:

    A spouse is the person to whom the primary insured person (employee) is lawfully married and named in the application when the employee first applied for coverage or added at a later date. Not more than one spouse may be insured at any given time.

    Dependent children include any natural children, stepchildren, grandchildren, foster children, adopted children or children placed as foster children for adoption who are: (a) unmarried; (b) younger than age 26; and (c) financially dependent on the primary insured person (employee). Dependent children are eligible from birth to age 25 (may vary by state).

 

The following other individuals are eligible for coverage under a Group Whole Life certificate:

    The actively employed person’s spouse. A spouse is the person named in the application when the employee first applied for coverage or added at a later date. Not more than one spouse may be insured at any given time.

    The actively employed person’s child or stepchild under 26 years of age.

    A child under 26 years of age the eligible person will be adopting pursuant to an interim court order of adoption.

    The actively employed person’s grandchild under 26 years of age, who is legally dependent on the actively employed person.
