Claims for the Health Screening Rider may be handled over the phone. The insured person should contact the claims department, as detailed in the About Assurity section, with the itemized bill (HCFA 1500 form and/or UB-04 form obtained from the medical provider) on hand to provide the following information:
•name and contact information for the medical provider;
•date of service;
•type of service;
•diagnostic code; and
•amount charged.
